How healthcare expenses are reported in payroll

Why healthcare payroll lines leave out the 31.42% social fee, and the Swedish context behind it.

Healthcare expenses get a specific treatment in the payroll report: the amount that lands on the payroll line leaves out the 31.42% social fee (arbetsgivaravgift), even though that fee was added when the expense was first created. This article explains why the healthcare line looks the way it does, what it means for the reimbursement and Förmånsvärde columns, and the Swedish context behind the rule.

What counts as a healthcare expense

CLVR treats an expense as healthcare when it belongs to the healthcare benefit category, or to the private healthcare insurance benefit. Those are the items that get the social-fee treatment described below. Everything else (wellness, home and leisure, pension and insurance, and so on) follows its own category rules.

The fee is added on the way in, then removed in the report

For a healthcare expense, two things happen at different moments:

  • When the expense is created, CLVR treats healthcare as a benefit-taxed (BIK) category, so the 31.42% social fee is included in the amount drawn from the employee's benefit allowance. The full allowance figure therefore carries the fee.
  • When the payroll report is generated, that social fee is stripped back out. The healthcare line shows the amount the allowance covered minus the fee, so the value on the file is the employee's actual reimbursement or benefit value, not the inflated allowance figure.

The result is the number you want on the payroll file: the employee's real reimbursement (or Förmånsvärde for benefit-taxed items), with no social fee baked in.

Note.

This is the same reason a healthcare line can be smaller than the figure deducted from the employee's allowance. The allowance deduction includes the fee; the payroll line does not. For the general version of this, see why a payroll line amount differs from the receipt total.

What this means in the report columns

On a healthcare line:

  • The reimbursement / Pris inklusive moms amount, and the Förmånsvärde amount, exclude the 31.42% social fee.
  • Pris exklusive moms is then derived from that VAT-inclusive figure by removing VAT at the expense's own VAT rate.

So healthcare lines are reported net of the social fee across the board, which keeps the payroll figures aligned with what the employee actually receives.

Why this is a choice, not a universal tax rule

Whether a particular healthcare arrangement should be benefit-taxed or exempt from social fees is not a single fixed answer in Sweden. It depends on the specific healthcare product, the employer's arrangement, and the tax interpretation each company is comfortable defending (for example, a private healthcare insurance versus company-paid healthcare services).

CLVR's current behaviour reflects the interpretation chosen with our first customers: healthcare lines are reported with the social fee removed. It is a Sweden-specific behaviour tied to the healthcare benefit identifiers, not a rule that applies to every benefit or every country. If your company needs a different interpretation for healthcare, raise it with your CLVR contact so it can be matched to how you report.

Troubleshooting

  • The healthcare line is smaller than the allowance deduction. That is expected: the deduction includes the social fee, the payroll line does not.
  • A healthcare expense is taxed differently than you expect. The social-fee treatment is set for the healthcare category and the private healthcare insurance benefit. If your company's arrangement should be handled another way, contact your CLVR contact rather than changing it per expense.
